The duration of your initial consultation appointment is thirty minutes.  Please ensure that you arrive five to ten minutes ahead of your appointment time, so that we have the maximum amount of time available to take a thorough history and to examine you.

We do need to collect some relevant personal information from you in order to provide the best and most appropriate treatment for your condition and this information will be kept strictly confidential.

Please bring the following with you to your initial consultation appointment:

• Referral letter from your dentist (if this was given to you)

• Signed Consent Form 

• Completed Medical History Form

• List or clear photograph of all current medications showing pharmacy labels

• Dental X-rays & scans

 

Please download the medical history/consent form and fill them out and bring them to your appointment.  If you have a complicated medical history or a long list of medications, we would be grateful if you would supply this information by providing a hard copy of a medical history summary from your family doctor.
